kernel, boot: improvements and tweaks on the kernel's filesystems.
- HeFS requires a 4gb disk at minimum now.
- make_app fully supports STEPS.
- Errata of NeFS.tex, add HeFS.tex.
- Better boot flow.
- New filesystems for FileMgr.
